An Ohio State Highway Patrol report states a semi rolled over several times on the off-ramp at the Ohio Turnpike Exit 25, Archbold- Fayette interchange about 6:05 pm, Tuesday, Aug. 12.

The report states Ronnie D. Stephens, 68, Colona, Ill., was driving the truck owned by Thompson Truck And Trailer, Dubuque, Iowa, east on the tollway.

Stephens told troopers another truck in front of his truck slowed.

He activated his brakes but they failed. He turned onto the exit ramp to avoid striking the truck ahead of him.

The Stephens semi went off the left side of the exit, struck several sign posts, and overturned several times before striking an embankment.

Archbold Rescue responded to the crash scene. After freeing Stephens from the wreckage, he was flown by helicopter ambulance to Toledo Hospital.

No information on his condition was available.

A passenger, Jerry Lee Hatch, 58, Davenport, Iowa, was taken to the Fulton County Health Center where he was treated and released.

Stephens was cited for operating a vehicle without reasonable control.
